Directions for E-Mailing Forms
- Fill out the appropriate form using the writeable version. Note that
you should only be able to type into the form fields.
- If you can write outside of the form fields, then you need to
go to the menu and click "View," click on "Toolbars," and choose the
"Forms" toolbar.
- You will see a new toolbar in your word document. Click
on the button that looks like a lock. That should lock the
document and you can then fill it out correctly.
- Save the form where you can find it (on your desktop for example).
- Click HERE to send
me an e-mail.
- Write "New Patient" in the subject line.
- Attach the saved form (that you, for example, saved
on your desktop).
- Send the e-mail.
