Download a release of records form if there are chart notes, labs,
imaging, etc at another doctor's office that will be needed for proper
care.
You will need to print it, fill it out, and send it to your other
doctor's office. In general, it is useful to get the last 3 years
of chart notes, labs in the last 3 years, and any imaging. Call
me if you have more questions about this.
